If you have any difficulties taking part or accessing the online or paper form, you can contact the Let's Be Heard team.

How to begin the online form

To begin the online form, click on the button at the bottom of the

web page that says “TAKE PART”. This will take you to the questions.

How to move on to the next question

When you have finished completing each question, click “SAVE &

CONTINUE” to go to the next question. You can go back to the previous

question if you want to.

How to submit your form

When you have completed the online form, a box will appear that says

SUBMIT”. When you select "SUBMIT" your form will be received by the

Scottish COVID-19 Inquiry.

Helping someone else to use the form

If you are helping someone else to take part, you can tell us on the form. All of the questions that follow address the individual concerned with words such as ‘you’ and ‘your’. Please ensure the person you are helping is aware of the questions. It may be helpful to read them out.

If you are submitting the form on behalf of someone else, please do not provide your contact details or those of the person you are helping. If you do provide these details, we will delete them.

You can also then complete your own online form to share your own experiences if you wish to.

Submitting on behalf of a group

If you are submitting on behalf of a group, please use the "group discussion" form. This can be found in the "Group discussions" page of our website.

Saving and returning to your online form

You might want to take a bit of time to write your response. You might also want to take a break part way through and come back to your response later.

There are two ways you can do this:

If you register with us you can use "Save and Return" to save your progress and take a break. You can register by using the register button on the site. You can find this on our Home page or on the Share your experiences page.

If you do not want to register with us, you can still complete the online form, but you will not be able to save and return. Instead, if you need a break, you could type out your answers to the questions in a separate document and then paste them into the online form.

Registering and data protection

If you register and then complete the online form, the information you share in the form will be linked to your registration details, including your email address.

Because of this, if you complete an online form and do not give consent for us to contact you in future, we will separate your answers from your registered account.

If you have any questions about this please contact us at LetsBeHeard@covid19inquiry.scot.

Our privacy notice explains how the Inquiry will use your information.

Changing your mind

If you choose to give consent to link your contact details to your response and later change your mind, you can let us know by emailing LetsBeHeard@covid19inquiry.scot and we will remove your personal contact details from your response and delete your contact details.

Taking part anonymously

You do not have to share your personal information with us to participate in Let’s Be Heard. You can submit this form anonymously without adding your contact details.
