First look at the questions on our Easy Read form.

Then record yourself answering the questions.

Say the question out loud first, then your answer to the question.

Do not say your name or your address unless you want us to know who you are.

Then save your recording on your computer.

Click on the purple "CHOOSE FILE" button below. It will ask you to choose a file to send to us.

Choose the file that is your voice recording.

Then answer our questions about keeping in touch on the next page.
