The questions

There is a short section asking for details about your business, including self-employment.
You will then be asked to tell us about:
access to financial support;
information provided to businessesrelating to the pandemic; and
key worker status.
You then have the choice to answer some optional questions about yourself. While these are not directly related to your business ownership, they will help us better understand the impact of COVID-19 and the public sector response to the pandemic on different people, which will improve our findings.
